The long distance between wire and box can, if you will, act like an antenna, picking up extraneous signals. In the last 25 years that I was working, I don't think I ever saw a unipolar pacemaker implanted, though I'm sure there are still a lot of them out there. Currently almost all implanted pacemakers are bipolar, with anode and cathode being close to each other at the tip of the wire.

I have a theory… Back when I was a college student, I noticed that the most screwed up people I knew of were all going to school for psychiatry and psychology. It wasn’t that there weren’t also well adjusted and normal individuals, but the number of people with issues was daunting… My theory is that at some point over the last 30 years or so, the lunatics have quietly taken over the asylum both figuratively and literally. They are now the ones that are defining what is normal and abnormal…
