Sometimes the artist group themselves flesh it all out in the studio. Tracks will get recorded that are for that purpose only, bits and pieces pulled from here or there get stitched together, the band puts it together, or the producers, the finalized idea gets learned and rehearsed, and then the real tracks get laid. Sometimes the suits say "hey, that sounds worth pursuing - send that to _______ and have it reworked."
