On the bright side, they all sounded like the same person behind the scenes as they do in front of the camera, more or less. What you see is what you get. I'm sure this mistake won't be repeated, so that's the last glimpse we'll get of these sorts of conversations. Until Woodward's next book comes out, of course.

It seems Waltz is the main person to blame for adding JG to the group chat. Others might not have thought to double-check each name in the group, although they should’ve..among many other national security communication concerns. Amateur hour.

In today's world, I suppose not too unusual. If on a group chat and you are accessing via mobile, you just see a couple of initials, and then +9 (for example) others.
Dont know if each of those people is familiar with each of the others, and if they did check, they probably just assumed is was someone in XX department.
(not to say this wasn't a bad thing, but I can kind of understand how it happened on a messaging app)

Someone made a ridiculously careless mistake, then everybody else understandably didn't bother verifying the attendee list. And of course, using that app to begin with is against the rules, and as it happens, that rule in this case, would have prevented this mistake. Since Goldberg is presumably doesn't have an account on approved messaging platforms suitable for military matters.
