      George - when might they try to extubate him? Now that he’s awake and all.

        @jon-nyc several criteria have to be met.

        Among them:

        • ability to maintain a reasonable SpO2 on a low FiO2 (probably about 0.4)
        • ability to maintain spontaneous respiration with endotracheal tube in place for a predetermined length of time - in my day, it was 24 hours - with no ventilatory support
        • ability to protect one's airway. Given that he's conscious, I doubt that this is an issue
        • assurance that his lungs have recovered from whatever insult they may have sustained (related to #1)
        • stable, or improving chest x-ray
        • assure adequate strength to maintain #2 and #3 (probably not an issue since his illness has been so acute and short-lived)

        #1 and #2 are the most important.

            @jon-nyc those were, more or less, the criteria back in the dark ages when I was a fellow on the ICU service (respiratory). I imagine the criteria haven't changed all that much.

                From CNN: "Here are the major takeaways from today's news conference at the University of Cincinnati Medical Center, where Hamlin has been treated since he collapsed Monday night:

                Hamlin has made "substantial progress": The player is demonstrating signs of "good neurologic recovery as well as overall clinical improvement," UC Health Dr. William Knight told reporters.

                It's still a long road to recovery: Doctors emphasized multiple times that Hamlin has a "ways to go." He'll need to start breathing on his own, and taking him off a ventilator will be a cautious, gradual process. UC Health Dr. Timothy Pritts said it's too soon to say for sure whether he'll make a full recovery.

                Hamlin is interacting with family: Loved ones at Hamlin's bedside have been able to hold his hand and communicate with him through written messages, though the player isn't speaking. Doctors said he even asked if the Bills had won the game.

                The cause of his cardiac arrest is not yet clear: The doctors said tests are still underway to determine what caused the medical emergency and that there are no "definitive answers" at this time.

                An outpouring of support: Doctors thanked community members for holding vigils, sending food to the hospital and lighting Cincinnati in the Bills' red and blue.

                    Just give both the Bengals and Buffalo W’s for last week. That way all of the other teams have to play the week straight up, the way they should. The only team it screws is the Ravens, and who really cares about that?

                    If the Bengals and Bills win this week and KC loses, give the Bengals the #1 Seed by virtue of head to head against KC and they had the lead and homefield advantage in the game against Buffalo…

                        So here’s the proposal

                        It essentially screws everyone but the Chiefs and Patriots.

                        If I made the decisions for the Bengals, I would take this week as my bye. There’s a small chance that you will meet the Ravens, and it would be better to be healthy and rested…

                            Competition committee voted last night. Owners today. The league and NFLPA have all been in on the conversation.

                              My utmost respect for them to fight through their existential despair at watching this young man nearly lose his life. Thinking about money and their livelihoods at a time like that must have been all but impossible. I know I wouldn't have been able to do it. I would still be in a fetal position if I were them, maybe in need of an IV feeding tube. Because I'm a good person.

                              Education is extremely important.
