There was a recent Ezra Klein podcast with Magie Haberman (NYT reporter on the elections beat) - and she's no fan of Trump, but she admitted that he's still mentally all there. She said he might have become a bit ramblier in his old age, even more tangents, etc.... but mentally he's been mostly the same guy that she's observed from the start.

It's a storyteller device. He's just getting older and sometimes the circle gets too wide.
