If the vaccine isn’t completely effective then it matters a lot if people choose not to get it, since they’re increasing the chance of infection for higher risk folks, as is the case with the flu vaccine.

During flu season, people at work were required to get flu vaccine. Those who couldn't or refused, were required to wear face masks if they wanted to work. I saw several nurses wearing masks while taking care of patients during flu season.
Simple choice: get vaccinated or protect others (which is, sort of, the same thing).

ObamaCare allows up to 50% surcharge on healthcare insurance premiums for smokers compared to non-smokers in the same age bracket and geographical area. Furthermore, even though smokers are charged higher premiums, they do not get any additional government subsidies to pay for those higher premiums. That said, while the "50%" surcharge is the maximum allowed by the ACA, individual states get to set lower surcharge levels, including no so charge if a state so desires.
