Cheaper, no legally protected doctor-patient confidentiality, presumably also no malpractice lawsuit or malpractice insurance. Actually I don’t know to reason about insurance needs for a “job” like this.

I went to a therapist a few times many years ago. The first thing they tell you is that if you say something indicating you intend to harm others, they are legally obligated to inform police.

Yep. He's someone there to listen. Not so dissimlar from a therapist.
If you want someone to pretend to care about your personal problems for an hour, you really do have to pay them.

There are a lot of lonely people there. Just having someone around can be comfort.
In addition, I could see a female hiring him if they wanted to walk somewhere but not by themselves.

Yeah, I don't think "do nothing" is really what his service is. More like "do nothing physical."

He’s an Emotional Support Human. He should have one of those vests on and should be allowed to ride in the cabin of the plane with his human.
