I didn’t view this as commentary about his specific NK policy, such as it is.
Having said that, I don’t think it’s clear whether his replacement will be better or worse.

It’s the transition period that has high odds of unstable consequences.

As if it’s a given she would be accepted, by let’s say the military? Coup perhaps or worse specific acts on her part to solidify her position? Or let’s say meltdown and what does China have to do on the peninsula and what about the security of the nuclear arsenal?

I think people are correct that his sister would be leader, maybe only in name. She has been promoted recently and has been named to the DPRK party central committee. However, the DPRK is still very "man-centered", so I am not sure that she would be completely accepted. Maybe as part of a ruling committee.
His uncle, who was second in command, but was executed by Kim Jong Un, has a wife who had disappeared from public, but has been back in official engagements recently. However, I dont think she has much remaining power.
There is a older brother, but he is supposedly gay and wants nothing to do with politics.
Premier Kim and his wife (Ri Sol Ju) also supposedly have three children, the oldest who is a boy who is about 10 years old.
So maybe there would be a committee to rule for 10-15 years until the older child is older.
But, even with all of that, i do not think much will change if Premier Kim is in fact dead or unable to rule.

I dont think so. Everything that I have hear about Premier Kim is that he did a good job of getting rid of people to make sure that those close to him are very loyal.
But, with DPRK, who knows!! LOL

If Kim Jong Un is holed up in his luxe seaside hideaway in the port city of Wonsan, he may have plenty of company — a harem of 2,000 sex slaves, a report said Friday.
The North Korean dictator resurrected his late grandfather’s “Pleasure Squad” of secret sex entertainers in 2015, The Sun reported.
And while the dictator’s location and the state of his health remain a mystery, some reports say he fled to a luxury resort in the port city located along the eastern side of the Korean Peninsula to avoid catching the coronavirus — and the UK paper speculates that he might have his harem with him.
“It’s within the walls of his ultra-protected palaces that his Pleasure Squad provides entertainment for high-ranking North Korean officials,” the paper reported.
“The 2,000-strong harem is said to be made to sing and dance for the nation’s elite — but also take part in bizarre sexual games and orgies.”
Kim is widely known for his love of booze and fine food — and the Pleasure Squad provides him with kinky carnal pleasures as well.
Some members are said to have been as young as 13 when they were taken out of school and forced into sex work.
Defectors from North Korea even say the girls have to undergo invasive medical examinations to prove they’re virgins before they can join.

North Korean leader Kim Jong Un made his first public appearance in 20 days as he celebrated the completion of a fertilizer factory near Pyongyang, state media said Saturday, ending an absence that had triggered global rumors that he may be seriously ill.
The North’s official Korean Central News Agency reported that Kim attended the ceremony Friday in Sunchon with other senior officials, including his sister Kim Yo Jong, who many analysts predict would take over if her brother is suddenly unable to rule.
State media showed videos and photos of Kim wearing a black Mao suit and constantly smiling, walking around facilities, applauding, cutting a huge red ribbon with a scissor handed by his sister, and smoking inside and outside of buildings while talking with other officials.
