Interesting. Lots of nuances with that rule too. Somewhat of a judgement call but if the batter doesn't try to get out of the way, it can be a ball. Or if the batter is hit but the pitch is in the strike zone, it's a strike. Or if the ball hits the batter's hands, it's a dead ball (and a ball or strike or foul ball, I think).

Do you have to hit the ball with the bat? For example, you try to hit it and miss, but the ball bounces off your shoe before hitting the ground and you run to the base.

No. You just have to swing. if you don't swing you take your base.
You also have to at least show an attempt to avoid being hit or it has to seem like there was unavoidable…
