Psychologists could write dissertations, disentangling why he chose to put his hand in front of the muzzle before pulling the trigger of what he thought was an empty gun.

Muzzle awareness, at all times. Had a good buddy that left a ricochet hole in an interior house door as a reminder. He was cleaning a .357 and thought he had emptied the cylinder. One round was stuck, though. When he cycled the gun and let the hammer fall, he shot his floor. The bullet ricocheted off the floor and through an interior door.
Good muzzle awareness, so nobody was hurt. A lesson learned the hard way, by one of the best men with a handgun I've ever seen.
