Huge pet peeve of mine. Corollary to that one is "Never enter a room when you're talking."
As someone who spent countless hours waiting for late surgeons, this hits home. I can understand emergencies delaying you at other places, but unless someone's dying, there's no excuse for not showing up at 7:30 for a 7:30 case.
We usually do Thanksgiving buffet-style. A while ago, we had some guests for TG. One of them loaded his plate with food, and was done eating before everyone was even seated.

The saying, "Lite is just one damn thing after another," is a gross understatement. The damn things overlap.

Etiquette rules only exist to create an environment where everyone feels comfortable and welcome. That's it, that's all they're intended to do. If a rule doesn't fulfill this requirement, then it's no good to anyone and should be ignored.
I'd say most of these are worth keeping, though.

Nonsense. A guest is someone invited or otherwise allowed on the promise on civil terms by the those authorized to decide who gets to be on the premise, typically excluding those providing goods and/or services to the establishment on the premise.

Yeah, being a guest under those conditions would be a really big whoop. "Did you have a good weekend?" "Sure. I was allowed on the premises of some friends, and I had a really . . . lukewarm time."

I worked with a surgeon who insisted that a 7:30 case meant the knife hits the skin at 7:30. Woe to the resident who didn't have the patient on the table, asleep, prepped and draped in time for a 7:30 incision.
"Good afternoon!" he would say to the resident. "I got our patient all ready. Why don't you go make some rounds while I relieve this man of his gallbladder."
