'Empty' COVID vaccines at VA Kroger. How can this happen?

I really cannot see any way that this happens.

Wow. Stories like this are why I really do not want to go to a walgreens, or cvs, etc. Some of my friends are starting to get inline for the "extra" doses left over at the last hour of the day.
Due to her medical issues, I want Mrs. Mark to be in a clinical setting when her vaccine is administered. She is calling them today and we are going to try to get vaccinated at the same time.

Seriously, what do they mean by given empty? Were they actually given shots? Shots of what? Were the bottles full of liquid but not the vaccine? Surely they would have noticed if the bottles were physically empty of anything?

19 defects in 100 million is PFG.
They didn't say how many were affected, The 19 in the link is referring to COVID-19...

So it was likely less. I’ll take a blind bet as if it were many the journalist would led with that.

Here's a better account. Fewer than 10 AND the needles were empty. The technician thought they had been filled and they weren't. Which is still bizarre. Were the plungers pulled? If so, then they were full of air... I guess that's okay if not injected into a vein or artery?

Shot is IM, so no problem.
I didn't watch too close, but seems to me the Pfizer is mixed before pulling it into the syringe. Maybe a mix-up that way?
