To play devil advocate, maybe some of the funds are for preventive medical care - checkups, etc.
Somebody has a USD$100 problem that can be cleared at a clinic. They dont have access to that and so they wait and wait and the problem becomes worse, and it becomes a USD$100K problem when they show up at the emergency room and cannot be turned away.

Not "devil's advocate" at all. From the perspective of public health, it truly is risky for the nation to leave pockets of populations vulnerable to communicable diseases.
