In NY you can get a special plate if you’re a physician. Not sure why anyone would want to do that.

In Asia, the numbers are the big thing. Certain number combinations are considered lucky, for example, #8 in CHinese culture means "money" or "riches". The goverment put these special plates up for auction, and i remember reading that in Hong Kong, a license plate with 8's put together (8888) sold for millions of dollars.
