In Philadelphia , a guy is in the hospital getting an IV of Remdesivir for COVID. One problem, the pharmacy intern mislabeled the bag and it was really cisatracurium, a muscle paralyzing agent. Hilarity ensues.

But, back to July...
I was very lucky. As a senior medical student I spent 12 weeks doing anesthesia at the local VA hospital. I was allowed to to just about anything short of major vascular. Everything from hernias to cataracts to bowel resections. I finished medical school 6 months early, in December.
So, come January, I started my anesthesia residency. I did it for 6 months. Come July, I was experienced AF - I could intubate, do spinal taps, run an arrest, start neck lines...so I was blessed having all that experience behind me come July. I had no fear, and as far as I know, I didn't kill anyone.

Then, in late October 2021, the leftover liquid from Epopa’s September 25 IV bag was tested, leading to the discovery of the critical error.
I suspect in many places this testing never would have occurred and the heart attack etc would have been labeled ‘of unknown origin’.
