the guy fell asleep, did someone try to call him on his phone? did the resident think, oh boy, here is my chance to work alone and didnt actively look for the attending? did he call another attending when the guy on call was not available?
i dont see such a tragedy and it could have been resolved if someone wanted it to.
anyway, i dont think the whole world should jump on this guy. if he was so tired that he went comatose in a car, he probably was too tired to operate anyway.
