Yup. It's a sternal retractor.
Here's an adult version:
I have to wonder how small an infant has to be for using that retractor. Smallest kid I ever put to sleep was on a Saturday morning in 1978. Weighed 750 grams (that's about a pound and a half). He had a PDA that needed ligating.

Looking back...
I remember back in 1980, anything much under 700 grams, we let die in the corner. We simply did not have the technology available in our part of the world to save them.
