      There are band-aids on both hands.

      Looks like he either had blood drawn from the hands or an IV inserted. In either case, I'd guess that the vein "blew" and he bled into tissue under the skin. Very common to see bruises like that in older patients with thin, delicate tissues.

                    Blood draws and IVs would not be done on a thumb. Those look like steri-strips on his right hand. And the bruising seems to end in a similar location on his fingers.

                            We were doing a portacaval shunt (a procedure done for cirrhosis). Surgeon got into some serious bleeding, and, because the patient's arms were tucked at his sides, I had no access for giving more blood in a hurry. I had the patient in a head-down position, and there was this big, fat juicy vein standing out on his forehead....
