Long distance surgery

I have to wonder what the circumstances surrounding this are.

Is there no local surgeon who can do a laparoscopic cholecystectomy? And yet, this, presumably remote, location has the tech of putting a fabulously expensive piece of tech in their OR?

I don't get it.

Also, at least when I retired, robotic chole was doable, but really offered no advantage over the standard lap chole.

High "Gee-whiz!" factor. Low practical use.
